Osteoarthritis of the Hip: Is it “Pain Because of Misalignment” or “Misalignment Because of Pain”?

Introduction

Patients with hip osteoarthritis often ask: “Am I in pain because my body is misaligned? Or am I misaligned because of the pain?”

The answer can actually be both. However, in most cases of hip osteoarthritis, the body tends to adapt by misaligning in order to avoid pain.


1. Typical Postures in Hip Osteoarthritis

Common postural patterns seen in patients include:

  • Anterior tilt of the pelvis
  • Increased lumbar lordosis (swayback)
  • Trunk leaning toward the painful side
  • Rotation or twisting of the knee

These are not simply “bad posture habits,” but rather compensatory movements to reduce discomfort.


2. Compensatory Misalignment Hurts if Corrected

It’s relatively easy to tell whether the misalignment is compensatory or the actual cause of pain:

  • If the pelvis is twisted and correcting it to the center increases pain → the misalignment is compensatory, adopted to avoid pain.
  • If correcting the misalignment reduces pain → then the misalignment itself is a cause of the pain.

In hip osteoarthritis, the former is much more common: pain comes first, misalignment follows.


3. Misalignment Caused by Pain (Pelvic Tilt, Swayback, Trunk Lean, Knee Twist)

Pelvic tilt, swayback, trunk leaning, and knee twisting are adaptive responses the body takes to avoid discomfort.

That’s why trying to “fix” the posture without understanding the cause can actually worsen the pain. Proper evaluation and guidance by a professional is essential.

The root cause of pain in hip osteoarthritis is bone marrow inflammation. As a result, patients often develop abnormal walking patterns and postural distortions. Instead of forcing the body into a so-called “correct” posture, it’s safer and more effective to consult a specialist.
