Introduction
When diagnosed with knee osteoarthritis (knee OA),
many people worry:
“Will running make my condition worse?”
However, recent research shows no clear evidence that moderate running accelerates joint degeneration.
In fact, it may even help improve pain and prevent further issues.
1. Scientific Evidence That Running Does Not Accelerate OA
The Osteoarthritis Initiative followed about 1,200 people with knee OA (aged 50 and older) for 4 years.
The study found no difference in radiographic progression
(such as KL grade or joint space narrowing)
between runners and non-runners.
Furthermore, pain did not worsen —
there was even a trend toward improvement.
2. Why Running Does Not Necessarily Cause Progression
- Cartilage Metabolism: Moderate mechanical load promotes nutrient circulation in cartilage.
- Muscle Strength & Stability: Running enhances lower-limb and core strength, reducing unnecessary stress on the knee joint.
- Balanced Cumulative Load: Although each running step carries higher peak force, the cumulative load per distance is similar to walking.
Together, these effects may help maintain knee health.
3. Caution for Those With Pain
A 2023 systematic review of 17 studies involving over 14,000 participants found
that recreational running was not associated with worsening symptoms.
However, extra care is needed if you:
- Have significant pain or swelling
- Have a history of ACL injury or meniscus surgery
- Perform high-intensity or competitive running
MRI studies also show that although cartilage thickness temporarily decreases after running,
it typically recovers within hours and does not cause lasting damage.
In Conclusion
For people with mild to moderate knee OA whose pain is under control,
recreational running does not appear to accelerate disease progression in the short to mid-term.
On the contrary, it may help relieve pain
and reduce the risk of needing knee replacement surgery.
Still, running without professional guidance can carry risks.
Always consult a physician or physical therapist
to find the safest exercise plan for your situation.
